The second point is the most damning, in my opinion. Here's an example:
1. Navigate to Bing Images.
2. Turn off safe search.
3. Search Bing images "sex"
4. Click on the second image.
At the top of the image detail page, there will be suggested search terms. One of them, for me, is "baby sex fetish"
They seem to process each search result image, and suggest other search terms based on what they think the image contains. For example, if I search for "breast", and click on a NSFW picture containing smaller-chested model, I end up with suggestions like "Skinny Small Tit Girls Naked".
For what it's worth, Google doesn't provide suggestions for NSFW searches at all. I think this approach goes a long ways to solving this specific issue.
Just searching for "sex" with safe search turned off results in child pornography search suggestions. You don't have to be looking for illegal content for this to surface.
